SIPOC stands for the five parts of any process: Suppliers, Inputs, Process, Outputs, Customers. It's a one-page "aerial photo" of a whole process — it shows where things start and end without getting lost in detail.
COPIS is the same diagram filled in backwards — starting from the Customer. You begin with "what does the customer actually want?" and work back to what must happen to deliver it. This keeps the process anchored to real value.

Waste (Japanese "muda") is any activity that consumes time, people or money but adds no value the customer would pay for. Lean identifies 8 types — remember them with the acronym DOWNTIME:

How to fill it in: for each type, write a specific example from your company (or leave it blank if it doesn't apply) and set the severity: Low / Medium / High. What you flag shapes the Lean tool recommendations in your report.

Process mapping means breaking a process into its steps and checking how long each takes. It's a simplified version of VSM (Value Stream Mapping), one of Lean's most important tools.
You classify each step into one of three categories:
%VA = the share of value-adding time in the total process time. In most companies it's just 5–20% — the rest is waiting and waste. It's usually shocking the first time you see it.
How to fill it in: add steps in order (name + time in minutes + type). 5–10 steps is plenty. The tool calculates %VA and total waste time for you.
